Sheraton Caregivers is seeking experienced, compassionate, dependable, and proactive Lead CNAs / Field Supervisors to join our growing team.
This is a hands-on leadership role for an experienced Certified Nursing Assistant who is ready to take the next step in their career. You will work directly in the field, supporting clients and caregivers, monitoring quality of care, conducting supervisory visits, mentoring staff, and helping ensure every client receives safe, dignified, and exceptional care.
Available Shifts:
Morning Field Supervisor Monday–Friday 7:00 AM – 4:00 PM
Late Field Supervisor Monday–Friday 3:00 PM – 10:00 PM
Every other weekend required

What You'll Do:
As a Field Supervisor / Lead CNA, you will:
• Conduct scheduled and unannounced in-home quality and supervisory visits.
•Complete new client check-in visits and ongoing quality visits for active clients.
• Observe caregiver performance and ensure services are being delivered according to the client's care plan.
• Monitor client well-being, safety, satisfaction, and overall quality of service.
• Use standardized quality checklists to identify safety concerns, caregiver performance gaps, and service issues.
• Provide real-time coaching and hands-on mentoring to caregivers.
• Support caregivers with retraining, recognition, guidance, and advocacy.
• Provide coaching in ADLs/IADLs, dementia care, fall prevention, infection control, and professional standards.
• Review and audit AxisCare care notes and visit documentation for completeness and accuracy.
• Identify missed or incomplete documentation and ensure corrections are completed promptly.
• Respond to urgent in-field situations and escalate safety concerns to leadership.
• Ensure incidents and critical concerns are properly reported and followed up on.
• Conduct client satisfaction check-ins and communicate concerns from clients and families to the appropriate department.
• Serve as a liaison between clients, families, caregivers, Scheduling, HR, Nursing, and Management.
• Document all supervisory visits, observations, coaching, follow-up actions, and communications accurately and promptly.
Required Qualifications:
• Previous supervisory, leadership, mentoring, or team lead experience strongly preferred.
• Active CNA Certification
• Valid driver's license required
• Reliable transportation and willingness to travel between client homes and field locations.
• 5+ years of caregiving or clinical experience preferred, including home care, assisted living, rehabilitation, or long-term care.
• Strong knowledge of ADLs/IADLs and personal care.
• Experience supporting clients with dementia and chronic conditions.
• Excellent communication, leadership, and interpersonal skills. • Strong problem-solving and decision-making abilities.
• Ability to work independently and manage multiple priorities in a fast-paced environment.
• Comfortable responding to urgent and changing situations in the field.
• Tech-savvy and comfortable using smartphones, tablets, and AxisCare/CRM documentation systems.
• Ability to complete accurate documentation following each visit.
• Highly reliable, organized, detail-oriented, adaptable, and accountable.
• Compassionate and able to build strong, supportive relationships with both clients and caregivers.
• CPR certification preferred.
